This program helps reduce isolation and increase meaningful social engagement for individuals who may feel disconnected due to age, disability, or health conditions. We often see clients who benefit from regular companionship, structured social activities, or community participation opportunities. Our caregivers and coordinators help connect individuals to local events, support groups, and recreational activities across Wisconsin communities. Emotional well-being is a key focus, as social isolation can significantly affect overall health. We aim to build consistent, supportive relationships that foster belonging and dignity.
Our program includes:
- Support groups (grief, recovery, parenting, etc.)
- Peer support programs
- Community outreach events
- Holiday assistance programs
- Back-to-school drives
- Food pantry partnerships
- Clothing & hygiene assistance
